The Complex Team is made up of ministry partners, regional authorities, and service providers with the aim of seeking collaborative options for support with children/youth (0-20 years old) with complex needs. Complex needs are defined as:
Having one or more conditions that severely impair their ability to participate in daily activities at home, school, and in the community
Require significant, extraordinary support and services from more than one ministry (Education, Health, Human Services)
Require planning and coordination of services because of the complexity and level of services required
Accessing the Complex Case Team is an opportunity to take a collaborative approach to discuss support. The expectation of the complex case team is to bring multidisciplinary partners together to collaborate around on-going support and identify a plan going forward as all systems are providing what they can and the youth/child is still in crisis. However, due to the complexity of the child/youth, the team may not always be able to provide immediate solutions.
Meeting Dates:
The third Thursday of the month is reserved for consultations or intakes with the team. New cases need to be submitted at least one week prior to this meeting.
