Accessing Services

Step 1: Before You Refer

For most students:

• Implement tier 1 strategies from the Continuum of Support Documents

• Reach out to your service provider (OT, PT, SLP) to set up a consult conversation or observation

• Implement strategies from this consult

Step 2: Fill Out the Forms

If a referral is deemed appropriate (after consult, or due to significant needs):

• Complete the referral form and have parents sign the consent form - this can be completed by the teacher or Learning Support Teacher (LST)

*Ensure forms are completed entirely with ACCURATE parent contact information*

Step 3: Submit Referral and Consent

• The LST submits the completed referral and consent forms to the division lead

All students with vision and hearing concerns should be referred to the Teacher for the Visually Impaired and/or Teacher for the Deaf and Hard of Hearing
