Reach out to your service provider (OT, PT, SLP) to set up a consult conversation or observation
Implement strategies from this consult
If a referral is deemed appropriate (after consult, or due to significant needs):
Complete the referral form and have parents sign the consent form - this can be completed by the teacher or Learning Support Teacher (LST)

The LST submits the completed referral and consent forms to the division lead
